News-style program exploring innovative ways of reaching a cohesive and coordinated care model
ALEXANDRIA, Virginia, March 14, 2016 /PRNewswire/ --
Of Americans aged 65 and up, 90% have one chronic illness, and 77% have two or more.
Such diseases are often preventable. AMGA launched a news and current affairs-style
program in partnership with ITN Productions which explores how to change the way health
care is being delivered from a reactive to a proactive care system.

'Transforming Chronic Care' aims to inform the sector and educate the wider public
about the innovative ways of improving collaboration within the healthcare community, as
well as of identifying those at high risk, preventing the onset of chronic conditions.
Introduced by Newscaster Johanna Botta, the program combines key industry interviews
and news-style reports with sponsored editorial profiles uncovering cutting-edge products
and services focusing on improving medicine practice with significant cost savings:
- EMD Serono demonstrates how they leverage real-world evidence to lead the way in
scientific innovation and patient centered-care.
- The Marshfield Clinic Information Services (MCIS) showcases how the optimized use of
information technology solutions focusing on workflows and modelling analytics benefit
both patients and physicians.
- MERCK describes the importance of integrated delivery systems (IDS) and how they
improve quality of care and population health.
"As leaders in the treatment of patients with chronic conditions, AMGA members will
benefit from this exploration of the issues and challenges of chronic care management,"
said Donald W. Fisher, Ph.D., CAE, president and chief executive officer of AMGA. "We are
excited to work with ITN Productions again, and we hope this project will further educate
the public and healthcare stakeholders on the innovative work being done to help patients
with chronic conditions live healthier lives."
Simon Shelley, Head of Industry News, ITN Productions, also commented: "ITN
Productions is delighted to launch Transforming Chronic Care with AMGA exploring the
importance of shifting to a more proactive approach to health care, and examining ways of
improving care for so many Americans. These stories can inform professionals, help engage
with each other and shape a promising future".
The program premiered at the AMGA Annual Conference on March 11th in Orlando,
Florida.
